Terms and Conditions
Client acknowledges and agrees to be bound by Biophysical's Terms and Conditions as stated below. Terms are subject to change at any time and without notice.
Cancellation requests will not be honored unless written or communicated through direct interaction with a Biophysical employee during business hours, 9:00am-5:00pm, Central Time Zone. No cancellation may be issued after phlebotomy has occurred.
Full refunds are offered only for cancellations placed within one business day of order, after which client agrees to pay $200 non-negotiable Cancellation Fee. Cancellation Fee may be reasonably raised should Biophysical or its partners have incurred additional costs, labor or otherwise.
Biophysical reserves the right to procure payment from credit card or other source at any time after order is placed. If credit card is declined or payment is otherwise delayed, client's test will not proceed until full payment is received.
A client who delays testing for any period of time is ineligible to retrieve any portion of their payment, but is not prevented from cancelling in accordance with Cancellation Policy.
Once Biophysical makes a reasonable attempt to inform client of phlebotomy appointment through phone or email communication, client will abide by the policies and guidelines therein. Client is subject to $200 fee if client fails to provide notice of intent to cancel or reschedule appointment, or if client's actions necessitate cancellation or rescheduling, within two business days of said appointment.
Client agrees that Biophysical Corp. may combine fees where applicable as deemed appropriate by Biophysical Corp.
