The cost of BiophysicalCheck is $995 for all services. You can order BiophysicalCheck by calling a Biophysical representative now at 866.968.0250.
During the call, we will take your order and set up your account. You will then be contacted by a Biophysical representative within 48 hours to schedule a time for a phlebotomist to draw your blood sample* at a convenient place for you. Or, you can complete this form, fax it to 512.623.4950, and a Biophysical representative will contact you to make the arrangements.
Biophysical accepts payment by VISA, MasterCard, American Express and Discover credit cards. Payment is expected in full at time of purchase.
Can I order BiophysicalCheck directly, or does my doctor need to order it for me?
Whether you can order BiophysicalCheck directly, depends on which state you live in. Please see the attached list for your state's specific requirements:
- In many states, you can order BiophysicalCheck without a physician's order.
- In other states, a physician's order is required. In some states where a physician's order is required, your results will be sent to your physician along with a copy sent to you. In a few states where a physician's order is required, you physician will receive your results and you can ask your physician to provide those results to you. If your state requires a doctor's order, you may print the Physician Order Form for your physician to sign and you or your physician can then submit the form to Biophysical Corporation.
