The Biophysical250 Order Form includes your personal information (name, shipping address, phone number, etc.) as well as your preferred method of contact. (A Biophysical250 representative will be in touch with you once we receive the form to gather additional information from you and help to process your order.) This form includes your billing information as well.

Medical History Questionnaire

The Medical History Questionnaire requests information about current medical conditions, prescriptions, immunizations, recent surgeries and hospitalizations, reactions to medications, current diet and level of physical activity.

Information gathered on the Medical History Questionnaire is used to make sure that your Biophysical250 results are as comprehensive as possible, so we ask that every client fill out this form prior to the release of their results. A Biophysical250 representative can gather this information from you over the phone, or you may print out a PDF version of the form, fill it out and then mail it to us. Once you've ordered your test and created a user account, you'll also be able to fill out this form online.

Informed Consent Form

The Informed Consent Form summarizes the Biophysical250 assessment procedure and ensures that you understand its benefits and risks. You will receive a copy of the Informed Consent for your review in the mail prior to your scheduled blood draw. Please read this document and call 1.866.968.0250 or email Biophysical Corporation with any questions that you may have. You can also print a copy of the Informed Consent. You will sign a copy of the Informed Consent at the time of your scheduled blood draw.
