This Privacy Policy applies only to information obtained through our Internet website.
I. Privacy Policy Summary
We use your contact information only to confirm your order, to communicate with you regarding your assessment, to mail or email Biophysical and biomarker news or offers of interest to you, or to answer any questions that you submit to us. Biophysicalcorp.com will never make any personal identifiable individual information available to third parties for third-party marketing, without your explicit agreement. Biophysicalcorp.com may share your e-mail address with a reputable e-mail delivery service to provide electronic messages that you had opted to receive from us.
We use a secure ordering process to ensure the safety of your credit card and personal information. Online transactions are handled through scripting designed to submit orders with Secure Sockets Layer (SSL) encryption. When you enter your credit card number and personal information into the Biophysical Order Form, it is transmitted across the Internet in an encrypted (scrambled) form, and then decoded once it gets to us. This provides a reliable and confidential ordering environment for your security. You can also order over the telephone, toll-free (866.968.0250) anytime or by mailing in an Order Form.
II. Information Collection
Non-Personal Information
If you visit our Internet website, but you do not order a Biophysical assessment, ask a question, or sign up for Biophysical's newsletter, the only information we collect will be Non-Personal Information through the use of Cookies. Non-Personal Information is not traceable back to you, cannot be used to identify you and includes information such as the Internet web pages that you have viewed. A Cookie is a small text file that is stored on the hard drive of a computer used to view websites that is accessible only by the party or website that placed the Cookie on the hard drive. Cookies enable Biophysical Corporation to track usage patterns and deliver customized content to users.
We collect these data on an anonymous, aggregate basis. We may also record the name of your Internet service provider. We use this information to measure site activity. We do not use information to track or record information about individuals.
Personally Identifiable and Personal Health Information
If you choose to order a Biophysical assessment, ask a question on our website or sign-up for the Biophysical newsletter, we require you to submit Personally Identifiable Information (i.e., information that can be traced back to you. Personally Identifiable Information includes information such as your name, address, telephone number and email address).
If you order a Biophysical assessment, personal information and information that identifies you as a Biophysical client are mandatory because that information is needed to set up your Biophysical account. Other information we request is optional. If you have any questions about mandatory or optional information, please call us.
When you register with biophysicalcorp.com, you are asked explicitly if you wish to receive the quarterly Biophysical newsletter, which will be mailed to your designated address, and monthly Biomarker e-News. If you do not wish to receive communication, you need to have the appropriate box checked. Some of the boxes may have been pre-checked for your convenience. If you do not wish to receive updates, correspondence or other offers, simply uncheck the corresponding box.
In accordance with the Children's Online Privacy Protection Act (COPPA), Biophysical Corporation does not actively market to children, and we never knowingly ask a child under 13 to divulge personal information.
III. Information Usage
If you order a Biophysical assessment the information collected by Biophysical Corporation will be used for the creation of your Biophysical account, aggregate data collection and demographic data, email communications and analysis of your Biophysical results. Users who provide information may receive email communications, information about their account and information about their Biophysical assessment results.
Other information collected (registration for newsletters, responses to surveys, questions, and the types of products you buy) is used to deliver the most relevant product information, news and offers to you. Survey responses are used only to improve our services and products and to help us better understand your usage, preferences, and attitudes. This information is used in collective form during our sales and normal business processes. We may ask you to supply optional information like your gender, age, and interests in order to help us better tailor our products and services to meet your needs.

Users may delete their information from our database by sending us a written request. We will delete your information and send you confirmation that the information has been deleted.
Note Regarding State Law
For all of the above purposes, in cases where state law is more restrictive than federal law, we are required to and will follow the more restrictive state law. For example, some states only allow physicians, and not consumers, to order laboratory tests, and other states prohibit a laboratory from releasing assessment results directly to a patient.

No general access is provided within or outside the organization. All pre-authorized individuals must provide a user ID and password match. Accessing your profile always requires both your member ID (e-mail address) and password.
Biophysical Corporation utilizes Secure Sockets Layer (SSL) encryption in transmitting Personally Identifiable Information to our servers, this includes information that you provide when you sign-up online for the Biophysical newsletter. To take advantage of encryption technology, you must have an Internet browser which supports 128-bit encryption.
Biophysical Corporation requires all Biophysical Corporation employees to abide by our Privacy Policy and be subject to disciplinary action if they violate it.
Biophysical Corporation backs up our systems to protect the integrity of your Personally Identifiable and Personal Health Information.
